Your Side is providing intake and referral, and program management for Out of Hospital Care, and holds the contract with Ministry of Health.
Community Services #1 (CS#1) is a community based not-for-profit organisation that has been serving the community for over 35 years. They manage and deliver a range of community services including community development, family support services, children’s services and aged care programs. Their purpose is ‘Connecting people to enrich lives.’ Their role is to provide case management and care co-ordination for Out of Hospital Care.
Our Approach:
Non-clinical Case Management is an integral part of delivering the OHC Program. It is key to ensuring that patients receive a package of care that meets their needs.
We use a Wellness and Reablement approach as a basis for our Case Management and home care service delivery. This means we aim to support people to increase their capacity wherever this is possible, with the aim of supporting people to remain independent for as long as possible. Case Management begins with the patient assessment and ends with case closure. Case Managers are guided by consumer directed care principles, they have the skills and experience to work with patients who are elderly, have chronic and complex health conditions, have sensory, functional, or psychosocial disability or are nearing the end of their life.
