ROLE: Community Services Consultants (Home Care Packages)
12 month full-time contract
We are looking for Community Services Consultants to work within our aged care team. Day to day you will be coordinating a number of aged care clients on home care packages enabling them to live in their own home independently, safely and with dignity. This will involve visiting aged care clients in their home, developing their care plans, coordinating their care, and managing their individualized budget.
Our offices are based in Chatswood, and this role will also have a community based component, which requires you to have a driver’s license. There is opportunity for flexible work arrangements including the potential for part-time or full-time hours, and working from home.
ROLE: Home Support Assessor
We are looking for a casual Home Support Assessor to undertake face to face assessments using the mandated National Screening and Assessment Form (NSAF), create Support Plans, and match and refer clients for a Comprehensive Assessment, service delivery or linking service support such as case management, assistance with housing, or non-aged care support. This role reports directly to the Manager, Regional Assessment Services. Please note this is a community based role, which requires you to have a drivers’ license and access to a vehicle.
