Home Care Package Referral Forms

It is important to register for My Aged Care Referral services like Home Care Packages before your situation gets serious and you need more support to remain living independently at home. Sometimes there are waiting lists for assessments or services – better to get the paperwork done early.

Register with My Aged Care over the phone (1800 200 422) or online to determine your eligibility and what funded program is right for you. My Aged Care will send you an assessor to complete a home assessment directly with you. This will determine which services are most suitable for you and best meet your needs. My Aged Care will then notify you of the level of support and services that you have been approved for. Find out more about what to expect from an assessment.

The My Aged Care assessor will finalise your approval for a Home Care Package, and issue something called a referral code. This means you are approved to have access to services. It’s important to understand that just because you have a code it doesn’t mean the assessor, or My Aged Care is organising services for you. You need to research and choose a provider, and then give them those referral codes from My Aged Care. Your chosen provider will then start to set up the services for you. You can search for providers on My Aged Care.
