At Your Side, two key professionals guide you through your aged care journey – our Aged Care Support Specialists and Home Care Package Care Consultants. Together, they ensure you are well-supported, from your first steps into finding aged care to ongoing care management.

Aged Care Support Specialists (ACSS)

What they do

Our Aged Care Support Specialists are here to ensure you are well-informed and supported as you begin your aged care journey. They act as a bridge between you and formal care programs such as the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), Short-Term Restorative Care (STRC), and permanent care. They aim to make your transitions through aged care as smooth as possible.

How they help

  • Helping you start your aged care journey: The Aged Care Support Specialist helps you navigate the aged care system, answers your questions, assists you with applications on My Aged Care, and prepares you for assessments like ACAT and RAS.
  • Advocacy: Your Aged Care Support Specialists can advocate on your behalf, ensuring you access the right services. Whether it’s transitioning from CHSP to HCP or organising reassessments, they work closely with you and your family to make sure your needs are met.
  • Interim support: While you’re waiting for your Home Care Package to be approved, your Aged Care Support Specialists can offer interim services, such as fee-for-service options, to keep you supported. They will also check in with you regularly to ensure the services you’re currently receiving meet your needs.
  • Setting you up with ongoing care: Once your Home Care Package is approved, our Aged Care Support Specialists will come to your home and do an assessment, gathering essential information about your health, environment and lifestyle to develop a Support Plan for the care services you need. The Aged Care Support Specialist will give you the budget projection and the Support Plan to approve and then introduce you to your dedicated Care Consultant who will provide you with ongoing support.

Care Consultants

What they do

Once your Home Care Package (HCP) is approved, your Care Consultant becomes your dedicated point of contact, managing your package and long-term care. Their goal is to ensure you receive the right level of support to live safely, independently, and comfortably in your own home, while making the most of the services available to you.

How they help

  • Tailored care planning: Your Care Consultant assesses your individual needs, preferences, and goals to keep your personalised Support Plan suited to your care needs and lifestyle.
  • Coordinating services: They arrange and oversee the delivery of all your care services, ensuring services are provided on time, effectively, and in line with your Support Plan.
  • Monitoring and adjustments: Your Care Consultant regularly checks in with you to make adjustments to your Support Plan and services as your needs evolve. This ensures you always receive the most appropriate care.
  • Advocacy: Care Consultants advocate for your rights and ensure you receive high-quality, person-centred care. They help you navigate the home care system, access additional services, and apply for relevant schemes, subsidies, and supplements.
  • Budget management: Your Care Consultant helps manage your HCP budget, ensuring your funds are used efficiently and appropriately for the services you need.
  • Compliance and documentation: They ensure all services comply with regulations and maintain accurate records related to your Support Plan and the care you receive.
