Aged Care Support Specialist
Full time permanent
There are two Aged Care Support Specialist roles within the Communications & Growth team for Your Side. They do outreach in the community to help inform, support and assist senior Australians in applying for aged care, and make referrals to community support. At present, these team members are doing the vital work of educating older people in the community about the aged care reform and Support at Home.
The Aged Care Support Specialists make client evaluations to assess their support needs and advocate for older individuals to get the appropriate solutions for their needs, as they guide them through the pipeline. They build trusted relationships, get to know each client’s challenges and goals, and conduct Home Visits to codesign a tailored Support Plan and sign them up to Your Side.
They work closely with our Home Care Packages Care Consultant team to brief them on each client’s needs and personality and maintain contact to check on their progress after their care is established.
They will work with a range of internal and external stakeholders to learn, inform and seek out opportunities to generate leads for clients. They have strong interpersonal skills, a love of conversation and high empathy levels, to establish rapports and influence decision-making. They will align to our values of respecting and safeguarding every client’s safety, dignity and interests, so they can live a good life.
